The healthcare team provides access to evidence based care, education and self-management support. Coordination of care will be provided by the healthcare team across multiple settings.

INTAKE

  • Please bring photo ID
  • Social Security Card
  • Current Insurance Card (if Necessary)
  • Referral (if Necessary)
  • Court / Probation paperwork (if Necessary)

Upon intake, a client financial situation is assessed to determine any possible available funding sources.
